What is BHH?
Broken Healer Home (BHH) is a person-centered, comprehensive care coordination program designed for individuals with serious mental health conditions. Our team works collaboratively with clients, their families, and healthcare providers to address a wide range of needs—mental health, physical health, substance use, and social support.
We serve as a central hub to ensure each person receives the right care, at the right time, in the right place.
Who Can Be Referred?
You can refer someone to BHH if they:
- Are diagnosed with a serious mental illness or emotional disturbance
- Are eligible for Medicaid
- Could benefit from whole-person care coordination
- Need support with managing physical and mental health conditions
- Are facing barriers to accessing consistent care
